We understand that this is a confusing time. Many things are rapidly changing and this may be causing you stress and anxiety about what to do for child care. We are here for you to help guide you through your options and ensure you do not lose your voucher even if you are not yet ready for your child to return to care. Please be in contact with your provider or your Family Services Specialist to fill out a Parent Enrollment Confirmation form and/or to discuss your options. In the meantime, below are forms helpful to families with child care vouchers.
- If you are due for a reauthorization, you will need to complete and submit the forms in the Client Renewal Packet.

- If you need to change your placement from one provider to another, you need to provide proper notice and can use the CCCB End Of Placement Form and the EEC Confirmation of Provider Form.
- If you have experienced a temporary change to your service need including but not necessarily limited to maternity or medical leave, seasonal work, reduction in hours, or loss of work during the COVID pandemic you should submit a Temporary Change Form.
- If you need to stop using your voucher for up to 180 days, you can request an Approved Break In Care Form. Your provider is not paid during an approved break and your slot at the program is not guaranteed to be held for when you are ready to return
- If your address, phone, email or other contact information has changed please submit an updated Parent Contact Information Form.
